The Fijians needed to win the match to clinch the title while the hosts needed just a draw after New Caledonia earlier defeated Solomon Islands 3-0.
The Carlos Buzzetti coach side suffered an early blow after goalkeeper Senirusi Bokini picked up a serious injury in the 16th minute and had to be replaced by Tevita Koroi.
Vanuatu scored its winning goal from the penalty spot through Captain Brian Kaltack in the 37th minute after a Fijian defender handled the ball while clearing the ball in desperation.
Fiji fought hard in the second half and striker Napolioni Qasevakatini was denied by the wood-work twice while Captain Rusiate Matarerega and Savenaca Nakalevu also missed from close range.
The Vanuatu National under23 side lifted their first ever Four Nations Cup in front of their own home fans in excess of 4000.
Meanwhile Fijian striker Christopher Wasasala scooped the Golden Boot Award while Solomon Islands goalkeeper Selonie Laruel won the Golden Glove Award.
New Caledonia won the silver medals while Fiji received the Bronze.
Solomon Islands were awarded the Fair Play Team of the tournament.
